SMA Guardian Parents,
The following includes important information regarding pre-participation documentation required before student-athletes can participate in practices or games. MSHSAA now allows for the physician portion of the physical to be completed every other year. The following documentation and testing need to be completed prior to athletic participation starting on March 2nd.
If your child already has a valid physical on file at St. Michael for the 2018-2019 school year only the annual requirements need to be completed and turned in prior to March 2nd.
If a physical for your child is not on-file at St. Michael both the two year and annual requirements will have to be turned in prior to participation on March 2nd.
In addition, student-athletes will need to return the following form with at least the concussion agreement signed.
To receive all athletic training services offered by the certified athletic trainers at SMA that portion of the agreement will also have to be completed.
St. Michael utilizes baseline concussion testing as part of the concussion policy. Testing is available to all student-athletes at SMA. Student-athletes participating in contact sports must have a concussion test for this school year completed prior to March 2nd to participate.
Spring Contact Sports:
- Girls Soccer
- Track & Field (Jumpers only; Hurdles, long, triple and high jump)
Testing can be completed at scheduled testing times for your child’s sport, that the coach will be in communication about or at scheduled times with our Athletic Training staff.
* If the above requirements were completed for a fall or winter sport they do not need to be completed again.
